should you wish to push through with the correction of the name of the passenger, it has to follow the procedure for name change with a fee.
please be advised that we may amend or change the name of the passenger provided that ticket must be rebooked to a regular fare since you availed a promo fare.
once fare is upgraded, kindly submit the following documents:
1 valid id (primary id) of the original passenger (note: in case of original passenger is a child, purchaser should provide such)
note: please attach a valid id of the child passenger, in the absence of the id please provide a copy of the birth certificate
1 valid id ( primary id) of the new passenger
pro-forma authorization letter (attached)- filled up by the original passenger (note: if the passenger on the booking is a child, purchaser should accomplish the form)
note: for child passenger, the name of the original passenger should also be indicated in the loa to avoid confusion**for international flight, kindly send a copy of the passport.
reminders:
name change should be done more than 4hrs prior scheduled flight.
primary ids are:passport (mandatory for regional flight), nbi clearance, driver’s license, sss/gsis id, prc id, postal id and senior citizen’s id
secondary -employment id, school id and other private organization-issued photo ids.
all ids submitted must be valid and should have a visible signature
name change is not applicable for bookings with promo fare.
once all documents are received and request is approved, name change fee must be settled more than 4 hours prior the departure date.
